Before you file a reimbursement claim: a document checklist
Most reimbursement delays come from missing papers. Collect these before discharge and your Claim File starts complete.
A reimbursement claim moves through four stages: Filed, Reviewed, Submitted, Settled. Most delays happen at the first step, because a document is missing. This checklist covers the papers commonly asked for. Your insurer, TPA or department may ask for more, so always check their own list.
Collect before you leave the hospital
- Discharge summary, signed by the treating doctor
- All final bills, itemised, with bill numbers
- Payment receipts for every amount paid
- Investigation reports (blood tests, scans, X-rays)
- Prescriptions and pharmacy bills
- Doctor's certificate or treatment details, if the insurer asks for one
Keep ready from home
- Policy document or health card
- Identity proof of the patient
- Bank details for the claim amount (a cancelled cheque is often asked for)
- For government-employee reimbursement: the forms and approvals your department requires
Simple habits that save weeks
- Ask for originals and keep a photocopy of each.
- Check names and dates match across every paper.
- Note the deadline your insurer or department gives for filing, and file well before it.
- Keep every receipt, even small pharmacy ones.
How Benlira helps
We prepare the Claim File, check it against the insurer's or department's list, submit it and follow up. We never promise approval, because the insurer or department decides.
